Workers' Compensation

Immediately report your injury / illness to your immediate supervisor or designated person in accordance with Administrative Policies and Procedures HR-44. To report a work-related incident, you must complete the Employee’s Report of Injury Form (PDF) along with your supervisor, who will call CorVel to initiate the claim. Your supervisor will complete the Employer’s Accident Report Form (PDF) and submit both forms according to County policy. All instructions for reporting work-related injuries and illnesses and supplemental information can be found in the Workers’ Compensation Accident Reporting Packet (PDF) .

Promptly report injury / illness to your immediate supervisor or designated person in accordance with your department’s protocol within 24 hours. To report a work-related incident you must complete the Employee’s Report of Injury along with your supervisor. The Employee’s Report of Injury can be found on page 13 of the Workers’ Compensation Accident Reporting Packet (PDF) .

For all work-related injuries / illnesses, you must choose a physician from the Panel of Physicians (PDF) | Specialists (PDF). The Employee Guide (PDF) provides information that will assist you through the process.

Medical Treatment

You will need to take a Medical Treatment - Physical Demands Analysis (PDA) (PDF) form with you to each medical appointment for your treating physician to complete. You must provide a copy to CorVel within 24 hours of your appointment.

Fire and Rescue Employees and Volunteers should use this Medical Treatment - Physical Demands Analysis form (PDF).

  • Treatment referrals must be authorized by CorVel in advance of your actual appointment.
  • Referrals and / or follow-up visits must be scheduled within 2 business days.
  • You must take the first available appointment offered to you. If you have difficultly obtaining an appointment, contact CorVel immediately.

You must present the CorVel billing contact information to the medical provider / facility to insure the correct billing. Failure to do so may result in you receiving medical bills directly.

Family & Medical Leave

Family and Medical Leave is job protected leave for eligible employees with certain medical conditions including work-related injuries and illnesses. FMLA runs concurrent with Workers’ Compensation and Injury Leave.
