How do I change my name?

• File in the Circuit Court of the county in which you live.
• Pay the $41 filing fee.

• It is suggested that these forms be filled out online and printed or typed for clarity.  If unable to do so, please print clearly and legibly.

How do I change the name of my minor child?

• File in the Circuit Court of the county in which the minor lives.
• Petitioners who desire to change the name of their child or ward must obtain consent of both parents to change a child’s name.
• Pay the $41 filing fee.

Please note: ALL applicants MUST sign the application in the presence of a notary or a Circuit Court Clerk.
