4. Pole Mounted Speed Display Signs - Maintenance Form

   The Department of General Services (DGS) and the Department of Transportation and Capital Infrastructure (DTCI) install and maintain Pole Mounted Speed Display (PMSD) signs to mitigate speeding and as part of Loudoun County’s Residential Traffic Management/Traffic Calming program and other requested initiatives. To track the location and maintenance requests of PMSDs, please complete the information below.

   Provide the sign location including cross streets, major roads and any other landmarks.

   Provide the number (e.g. PMSD 13) adhered to this sign.
