Restricted Licenses

Restricted License Forms

  • If the court has previously granted you a restricted license permit, you may come into the clerk’s office, Criminal Division, and fill out the Restricted License Application Form (PDF). The license will then be typed up by staff. You will need to review the typed license and sign. The form will then be forwarded to a judge for review and entry. Please note that this may take a few days to a week for entry. If you provide a phone number, we will call you; otherwise we will mail the restricted license upon entry.

 Granting Restricted Licenses

  • If the court has not granted you a restricted permit, you must place this matter on the Criminal Motion’s Day docket to go before a judge and have that privilege granted.
  • You must complete a praecipe form (PDF) to place this matter on the docket. You must also complete and file a Restricted License Application Form (PDF). If filing a praecipe a motion must be filed as well for the court to know what issue you are asking them to rule upon.
  • The praecipe form must be received at least one week in advance of the hearing date. The Criminal Motion’s Day docket is the fourth Friday of every month, except the fourth Friday in November or December.
