Affordable Dwelling Unit Program Required Annual Anniversary Affidavit

Homeowners participating in the Affordable Dwelling Unit (ADU) Program are required to complete an Annual Anniversary Affidavit no later than June 1 of each year they own their home. The original affidavit must be mailed to the Department of Housing and Community Development. Copies, faxes and emails of the affidavit will not be accepted.

Please note: If you are not able to have your 2022 ADU Annual Affidavit notarized by a notary public, please mail your original, executed affidavit and staff will notarize the affidavit for you.
