1-918 Supplement B

The Loudoun County Sheriff’s Office accepts U-Visa applications from law firms and individuals who may qualify for a U-Visa as a crime victim under federal guidelines. All U-Visa applications that meet the mandatory federal requirements will be reviewed by department personnel.

In order to be reviewed, the following items must be included in the packet:

  1. The I-918 supplement B form must be filled out completely, truthfully, and accurately.
  2. Applications must include copies of all supporting documentation to include birth certificates / child support paperwork, if indirect, marriage licenses, report numbers and court paperwork to include the criminal complaint and / or warrant(s), if applicable.
  3. A cover letter requesting a review of the application and certification must be included.
  4. U-Visa application form I-918 Supplement B will be accepted by hard copy only (mailed or hand-delivered).

Incomplete Applications

Incomplete applications will not be reviewed and will be returned to the sender.
